Completed applications can be dropped off at Mr. Furticella’s office in the Anderson building office 331, or mailed to the address below. A copy of your Purdue Calumet student ID needs to be included if you are applying for the INCPAS.
For the FAC application, you must include your student ID number. The cost for the club is $10.00 a semester.
